(chapter51)Malaria with persistent high fever

Patient Background
The patient was the daughter of Grand Preceptor Yu (a high-ranking official), over 80 years old, previously in robust health. During the transition from summer to autumn, she suddenly contracted malaria (a periodic fever-causing infectious disease).

Zui Hua Chuang Medical Cases Chapter 51

Symptoms Observed

  • Eyes as red as rubies
  • Extreme dryness of the mouth with cracked lips
  • Constant requests for cold water
  • Urine dark red, resembling blood
  • No bowel movements for over three days
  • Pulse taut like a tightened string and abnormally rapid in all six pulse positions

Diagnosis
I explained to the family: This is a classic case of heat-toxin malaria. Characterized by high fever without chills, it requires clearing excessive internal heat. Simply using “malaria-blocking agents” (medications to suppress malaria attacks) would prove ineffective.

Treatment Plan
Administered a high-dose Baihu Tang (White Tiger Decoction)—a classic heat-clearing formula:

  • Primary herb: Gypsum (Shigao) at approximately 50 grams (converted from the ancient measurement unit liang)

Treatment Outcome

  • Fever subsided after two doses
  • Malaria fully resolved after four doses
